All Athletic training forms must be completed online BEFORE August 1, 2017. It is required that all athletes read both fact sheets that are included in the athletic training forms (sickle cell trait and concussion). Please send in copies of your insurance cards with your paperwork in addition to proof of sickle cell trait results (if you do not sign waiver). Forms must be filled out and completed online in order to participate in intercollegiate athletics.
*Please note that the Bethany College requires all Freshmen and transfers students to have a physical before the start of classes. Please use the form sent by the Bethany College for the purpose of athletic participation. Any other form or format used by your doctor’s office may not be acceptable for sports participation at Bethany College. If you are using some other physical examination form, the physical must be completed no later than 6 months prior to beginning a sport and it must state the exam is for the purpose of athletic participation for the sport in which the student will be involved. Send all physical forms to the Bethany College Student Health Center
